For Marching Band Band Camp Lunch Order Form (doc) To be filled out by students to order lunches during band camp if lunches are being brought in. Physicals

Physicals and parental consent forms are required in order for students to participate in marching band and color guard. Students who could not attend the mass physical event on April 14th 2010 you may phone the Resurgens Fayetteville office at 770-461-1238 to arrange an appointment. Resurgens will still honor the $10 fee. Students need to bring with them a signed parental consent form and the required physical form posted below, as well as the Resurgens Cover Letter.

Resurgens Cover Letter (PDF) must be signed by parent.
Pre-Participation Physical Evaluation Form (PDF) A new physical is needed each year.
Fayette County Parental Consent Form (PDF) Parents must give consent for students to participate in sport activities. This form MUST accompany the "Pre-Participation Physical Evaluation Form" in order for students to be eligible to participate in marching band and color guard.
Consent, Acknowledgement, and Release Form (PDF) - Parent must give permission for overnight trips, such as Band Camp and participation in pool activities.

All marchers must have a current physical on file at MHS BEFORE they can attend the summer marching band camp. Students who prefer to visit a private physician must ensure their physician uses the required forms above. If a band student is participating in another athletic sport as well as marching, only one set of completed forms is needed.
